Instruction underspecification refers to user instructions that omit critical details — such as intent, target, or scope — needed to complete a task correctly. Research on agentic systems finds that when faced with underspecified instructions, models overwhelmingly default to guessing a plausible interpretation and proceeding rather than pausing to ask a clarifying question, which drives many real-world task failures.
